A patient with the typical clinical, roentgenographic and pathologic features of tuberous sclerosis is reported. Pulmonary involvement was extensive, a rare finding in tuberous sclerosis. The pulmonary involvement is indistinguishable from other causes of interstitial fibrosis. However, bony changes of sclerosis, periosteal new bone formation and cysts are almost invariably associated with the pulmonary lesions. Conversely, intracranial calcifications are rarely associated with pulmonary involvement. The complete clinical syndrome may not be present in all patients with pulmonary involvement.
